How It Works

To be considered, players must be registered with SLVYHA for the 2025–26 season.

Families should review the requirements below before applying. Equipment assistance is limited and awarded based on need and availability.

• Fill out the application form below in full
• All applications are reviewed confidentially
• Families will be notified by email regarding the status of their application
• If selected, equipment assistance will be provided based on available resources

Applications close December 30, 2025 or until all  assistance has been awarded.

Family Commitment

As part of receiving equipment assistance, families agree to:

• Volunteer 2 hours per equipment assistance recipient at SLVYHA home games
• Help secure at least $100 in sponsorships per recipient to support the program

If a family is awarded registration assistance in addition to equipment assistance, the following apply:

• Fundraising requirement increases to $200 per recipient
• Volunteer requirement increases to 4 hours per recipient

*If requirements are not met, you wont be eligible for future assistance.
**Families who receive this assistance are not eligible for the multiple player discount.
